You can place an order by fax or mail by following the instructions below:

1. Print out and complete our Order Form (PDF: 68 kb). No other forms are necessary if you have previously ordered.
2. Simply fax or mail us the completed order form, include a copy of your original prescription(s) if necessary. To guarantee the timely processing of your order, all prescriptions should be submitted in English. The authenticity of all prescriptions will be verified.
3. You may be contacted by one of our representatives to confirm your order and you will be asked to pay via MasterCard or Visa over the phone if you have not included a cashier's check, international money order, or bank money order with your mailed-in prescription.
